Prospective Trustee Application Form
Thank you for your interest in becoming a trustee of The PFC Trust. Trustees share collective responsibility for the charity’s direction, resources, compliance and public benefit.
Please answer in your own words. If you need this form in
another format or require a reasonable adjustment, contact the Trust before submitting.
Before you apply
Read the Trustee Role Description, Trustee Code of Conduct and current recruitment information supplied with this form.
Complete every mandatory section. You may attach a CV, but it does not replace the answers requested here.
Do not include unnecessary sensitive personal information. Eligibility and conflict declarations will be checked before appointment.
Appointment remains subject to the governing document, satisfactory checks, references, trustee approval and any induction requirements.
Section One
Applicants Information
Section Two
Motivation and connection to the trust
Section Three
Indicate your current level in each area. The Trust does not expect every candidate to cover every area.
Use the evidence section to describe your strongest contributions.
Section Four
Board contribution and practical commitment
Include how you handled challenge, disagreement, incomplete information or a decision you did not personally favour. Suggested maximum: 300 words.
Section Five
Eligibility, independence and conflicts
Please answer every question. A “yes” answer does not automatically prevent appointment unless the law or governing document requires it; the Trust may ask for proportionate further information in confidence.
Please use this section to add detail for any declaration made above, identify the question and provide enough information for a confidential assessment.
You may instead request a private discussion with the Chair or Governance
Lead.
List relevant employment, offices, business interests, trusteeships, memberships, close relationships or other loyalties. Write “None” if there are none to declare.
Section Six
Safeguarding, conduct and checks
I will follow the Trustee Code of Conduct and maintain confidentiality.*
I will complete safeguarding and other required induction or training.*
I consent to proportionate identity, eligibility and reference checks.*
I will undertake a DBS check if the assessed duties are legally eligible and the Trust requires it.*
I will promptly disclose any relevant change before or during appointment.*
I understand that trustee appointment is not guaranteed by submitting this form.*
Section Seven
References
Provide two people who can comment on your suitability. At least one should normally be a current or recent professional, voluntary or community contact. Neither should be a close relative.
The Trust will normally seek consent before contacting them.
Section Eight
Is there anything the Trust should do to make the recruitment process accessible to you?
This information will be used only to arrange appropriate support and will not be used to assess your suitability.
